Verify Coverage with The Lactation Network (accepting MOST United Healthcare (UHC), Aetna, Blue Cross Blue Shield of Illinois and Rhode Island, First Health, First Choice Health, Imagine Health, Meritain (select states), Multiplan, PNOA, Sutter Health, UHC, VA CCN - Optum East (regions 1-3), and Velocity.)

*Please note, office and virtual consults if coverage is verified are 100% covered, but there is an out-of-pocket minimum $150 travel fee with insurance coverage for home visits.

A superbill can be provided to you at the end of our session for you to submit to your insurance company to request reimbursement.

There is a $50 cancellation fee for all appointments cancelled within 24 hours of appointment time.
