Consultations may be covered by your insurance for lactation, infant feeding support, and prenatal and postpartum care!

After booking an appointment, you'll receive a secure insurance verification form by email.
Please fill out all intake forms including pictures of benefits card. Benefits are not able to verify coverage until forms are submitted.
It takes 24 business hours to verify coverage.
The fastest way to verify your insurance coverage is to schedule an appointment and complete the insurance verification forms sent after booking. This allows us to begin verifying your benefits right away while reserving your appointment time.
If you have questions about your insurance coverage before scheduling an appointment, please complete the Insurance Verification Request form, and we'll verify your benefits before you book.
*Please note, office and virtual consults if coverage is verified are 100% covered, but there is an out-of-pocket minimum $100 travel fee with insurance coverage for home visits.

If your insurance plan is not covered via these options, it does not mean your insurance will not cover all or part of lactation services -
A superbill can be provided to you at the end of our session for you to submit to your insurance company to request reimbursement.
Call your plan and ask about code S9443 with diagnosis Z39.1 (“care of lactating parent”). Even if it isn’t fully covered, part of your visit may be.
